US invoice template
There is no single national invoice format in the United States. What you include depends on your business, your state and your client. This template opens set up for US dollars and Letter paper, with sales tax left for you to fill in.
What an invoice in the United States usually includes
- Your business name and address
- A unique invoice number
- The invoice date and the due date
- A description of the goods or services, with quantities and prices
- Sales tax, if you are required to charge it, shown separately
- The total due, and how to pay

Do I have to charge sales tax?
It depends on your state and on what you sell. Rules for services and goods vary. Check with your state tax authority, or ask an accountant.
Will my client ask for a W-9?
US clients often ask independent contractors for a W-9 before their first payment, so they can report what they pay you. It is separate from the invoice.
Is anything I type sent to a server?
No. The document is made inside your browser. Your business details and the last document are remembered on this device only, so you do not have to retype them, and you can wipe them with the link under the preview.
